Member Portal Documentations and Forms

Now that you have finished setting up your account, let’s review each piece of the Member portal.

From the Dashboard, there are two ways to navigate the Membership Portal. You can either click a button within the blue tiles or you can click a button within the menu navigation at the top of the page. 

The “documentation” tab contains a handful of forms that need to be completed in order to complete before you can participate in the Students In Action program. All Students In Action teams must complete a Memorandum of Understanding. This form must be completed by the team advisor and administration. All program participants, including youth, advisors, and coaches must also complete a Marketing and Liability release. If you are under 18, this form must be completed with a parent or legal guardian. The Opt-Out Form is for youth and it is optional. Please review this form with a parent or legal guardian and decide if this is something you would like to complete.
